Медико-психологічна допомога та реабілітація військовослужбовців – учасників ООС

Abstract

Професійне супроводження фізичного та психічного здоров’я особистості є важливою проблемою у будь-якій галузі людської діяльності. Особливого значення ця проблема набуває у військовій сфері в умовах бойових дій. Мета статті – вивчити патогенетичні механізми соматичних та психічних порушень, що призводять до формування психічних розладів у військовослужбовців, які брали участь у бойових діях, та розробити на цій основі систему медико-психологічної допомоги та реабілітації військовослужбовців – учасників операції об’єднаних сил (ООС). Визначені основні патогенетичні механізми впливу отриманої бойової соматичної травми на психічні стани. Запропоновані принципи реалізації заходів медико-психологічної допомоги та реабілітації, що здійснюються після виведення військовослужбовців з району проведення ООС. Встановлені патогенетичні механізми соматичних та психічних порушень, що призводять до формування психічних розладів у військовослужбовців, які брали участь у бойових діях. З’ясовано, що успішна реконвалесценція та психічна адаптація цих військовослужбовців потребує системи медико-психологічної допомоги та реабілітації. Розроблена система медико-психологічної реабілітації військовослужбовців – учасників ООС.Professional support of physical and mental health of the individual is an important problem in any field of human activity. Particular importance of this problem becomes in the military sphere in combat conditions. The military-men of the Armed Forces and the National Guard of Ukraine are subject to a number of powerful psycho-traumatic factors due to the threat to their lives, life of their comrades, fear of the future, and the experience of the fate of their relatives. This leads to numerous psychogenic losses. Even in the absence of physical injuries and somatic diseases, the adaptation of military-men who return from the combat zone to peaceful life is complicated by numerous socio-psychological problems, which actualizes the need for adequate psychosocial rehabilitation. The aim of the study is to define the pathogenetic mechanisms of somatic and mental disorders, which leads to the formation of mental disorders among military-men who participated in military operations and developed on this basis systems of medical-psychological help and rehabilitation of military-men – united forces operation (UFO) participants. Such basic pathogenetic mechanisms of influence of the received military somatic trauma on mental states were defined: direct effect of injury on the functioning of the brain; chronic pain; long-term medical treatment; negative assessment of the severity of his physical condition. Principles of implementation of medical-psychological help and rehabilitation measures, carried out after the withdrawal of military-men from the area of UFO conducting, were proposed: the earliest possible beginning of medical-psychological help and rehabilitation of military-men; obligatory psychodiagnostic, psychophysiological and medical examination with information on the conditions and forms of rehabilitation measures; complex and shared use of therapeutic effects; an individual approach to the choice of medical-psychological measures taking into account the degree of severity of mental disorders in a patient; sequence of medical-psychological measures; correction of medical-psychological help and rehabilitation; carrying out of psychoprophylaxis after the end. The established pathogenetic mechanisms of somatic and mental disorders that lead to the formation of mental disorders in military-men who participated in military operations include the following factors: mental and physical overload, possible somatic illness, acute mental stress and delayed psychogenic factor. It has been determined that successful reconvalescence and psychological adaptation of these military-men require a system of medical-psychological help and rehabilitation. The developed system of medical-psychological rehabilitation of military-men – UFO participants should be consistent and phased.
